Polycystic ovarian syndrome is occurring mostly in reproductive age women with the ovarian expression of various metabolic disturbances. It is a hormonal disorder causing small cysts developed on the ovaries.

Pathogenesis

Polycystic ovaries develop when the ovaries are stimulated to produce excessive amount of androgenic hormones.in particular testosterone by either one or a combination of the following.

The release of excessive luteinizing hormone {lh} by the anterior pituitary.

Through high levels of hypoglycemic agent in the blood in women whose ovaries are sensitive to this stimulus.

Multiple ovarian cysts are actually in mature follicles not cysts. The follicles have developed from primordial follicles .but the development has stopped due to disturbed ovarian function.

The follicles appeared as a string of pearls on ultrasound.

Mostly PCOS women have hypoglycemic hormone resistance and obese. Their elevated hypoglycemic agent levels leads to abnormalities in hypothalamic-pituitary-ovarian axis leads to PCOS.

• Congestive– Pain is usually dull, deep in nature and begins three to seven days prior to menses and relieved by the appearance of flow
• Spasmodic-Pain is intermittent, starts at the onset of menses and lasts for the first two to three days of menses.
